***REVISION FOR 2007***

If you require revision services for your horse(s) in 2007, you will need to act immediately and fax the following information to CAPREH president, Carlos Lara, at (780)  842 - 4099:

 Name of the Horse
 Horse's Date of Birth
 Horse's code or number
 Horse's Microchip number
 Name of the breeding farm
 Name of the breeder or representative
 Breeder's number
 A copy of the horse's Carta
 
The deadline for submission of this information to the CAPREH office is June 20th.  

If you haven't already sent in the INITIAL FARM REPORT, you will also need to include it.  The Initial Farm Report is a mandatory document which needs to be submitted in order to be eligible and receive ANY Services pertaining to your PRE horse.  The form, which requires the Breeder's signature, should be forwarded as soon as possible, along with a copy of the green portion of the passport and proof of purchase for each horse listed.

Before CAPREH can give breeders requiring revision services a total cost estimate for the 2007 service, we will need to know how many horses require revision and the locations throughout Canada in which they reside.  After June 20th, we will have a better idea on the total cost per horse and will relay it to all breeders requiring revision services.
